Pre-appointment details
Before your visit, please acquaint yourself with the location where services are offered and ensure you are going to the correct location. It is essential to fill out your intake forms prior to your appointment. If you are unable to complete the forms, kindly ensure that you arrive 10-15 minutes early for better service and care.
I require a credit card to hold your appointment.
Please familiarize yourself with our cancellation and no-show policy.
Cancellation and No-Show Policy
Your appointments and well-being are very important to me. I understand that sometimes, unexpected delays can occur, making schedule adjustments. If you need to cancel your appointment, I respectfully request at least 48 hour notice.
Any cancellation or reschedule made less than 48 hours will result in a cancellation fee. The amount of the fee will be equal to 50% of the service. Missed appointments will result in the same fee.
If you are more than 15 minutes late for your appointment, I may not be able to accommodate you. In this case, the same cancellation fee will apply. I will do my very best to reschedule you for another time that is convenient to you.
Cancellation and No-Show fees will be charged to your card on file.

We accept Debit, Visa, and Mastercard.
At this time, we are unable to direct bill.
